Last week, Teacup went in for her 2 year check-up so I decided it was high time I update my medical records for the girls. I’d been keeping a little journal but they were getting too bulk to luge around. The result–I decided to create a one page medical form I could include in my purse or the children’s backpack for easy reference. I love it! Now I can easily pull it out, find the information I need, add whatever new information our doctor shares with us, and even compare heights and weights of children which is just sometimes fun to do. That said, today I’d like to share a copy with you in case you would like to do the same. I have included the medical form as both a pdf document and a Word document so you can edit it to your heart’s content depending on how many children you have and/or how old your children are.
The chart includes a place to put your contact information, insurance information, medical history, and more. There’s a weight section so you can keep track of your child weight from well-child check-up to well-child check-up.
There’s a height section for you to keep track of your child’s height from check-up to check-up as well.
And finally, there is an immunization section for you to keep track of your child’s immunization records. On our personal records I also added the times in which the shots are to be taken in parenthesis (example: 2 months, 4 months, 6 months, and 15 months) next to the shot description so I can make sure we are on track according to our pediatrician’s recommendations or check to see if there are any shots needed at the next check-up.
The nice thing about it being offered in a Word document too is the fact that you can add extra columns for more children, or delete a column if you only want the form for one child or you can add more rows if your children are different ages and so forth. If you are not sure how to add a column or a row to a table a Word document, right click on the row or column you want to add too, select “insert” and where you want the column to be inserted.
